You receive a key to street door and 4th floor door and your room. there is a solid working elevator for luggage and quick trips up and down to street. Owner meets you there at check in and check out so verify arrival time but he only lives a few blocks away. Maid service daily to clean. Sheets and towels only changed 2x a week.
You are free to come and go as you please. He has a binder with a list of everything you will need information in each room, how many business is from meters from the Hotel, addresses and phone numbers from restaurants, bars, Laundromats, transportations, etc.. what to see and do in Rome everything you will need which is nice. Nice place for a Vatican or Museum visit as within 15 minutes walk plus a few blocks from metro station to see rest of Rome. One of Rome's few organic grocers is litterly next door and each room has a frig, with central use of a coffee maker, tea pot and micro.
Only major downside was thin walls from other rooms but most are quiet and don't expect privacy for couples. street noise and beggars in front of grocery (which is all of Rome) and hustlers on street in am are loud and annoying if you want to sleep in. as windows are old and drapes sheer, so If you get up early no problem. A clean room with private bath at a good price if you are traveling on a budget.
Stayed in hotels on both sides of river, east side of river has the majority of sites more touristy and Basilicas and a younger hipper vibe. West side is more business.
